I want to know how can I add one-time cost to a health state that a patient incur it only once they enter the markov health state? Do i need to use tunnel state?

If you are entering a one-time cost to a Health State when the cohort or trial enters the Markov model, you need to use the Initial Rewards (if you are using Half Cycle Correction) or use the Start-Up Rewards (if you are using Within Cycle Correction).
These are documented in the Help files for 2018 R1.0, section 36.3 details how to add Health States and State Rewards.
Aside: Tunnel States are used when the amount of time in a given Health State is important. For example, the amount of time you are in Metastases may impact the transition probability to Death from that state.

Note that you can also add one-time costs at any event node to the right of the health states via "transition costs". This would typically be used for costs associated with an event (e.g., hospitalization) that would not be appropriately applied to everyone starting the cycle in a specific health state.
I am having a similar issue. I have transitional cost associated with treatment and not with the health state. However, when I look at the extended report for the Markov model it looks like that cost is not being applied? Any ideas on how to troubleshoot this?
In the Markov Extended Report, every event node in the transition subtrees should be shown when the report is fully expanded. Please note that the default presentation shows only the health states until you expand the groupings within the health states. There is a full expand/collapse of all groupings available in the report header.
